Thanks to online forms, the information we gather is structured, can be delivered electronically, and used in other systems.

User Need

To complete a step, typically a starting point, in their journey to accessing a service online.

Success Criteria

The form is easy to fill out and on its own, provides enough context; cannot be just a set of fields. Must have clearly defined ownership of what happens to the data collected.

  2. Capture what data you want to collect – name, address, date of birth, contact information… What else? Be very specific.
