Forms are used to gather information needed to provide our constituents with a service.

Thanks to online forms, the information we gather is structured, can be delivered electronically, and used in other systems.

User Need	To complete a step, typically a starting point, in their journey to accessing a service online.
Success Criteria	The form is easy to fill out and on its own, provides enough context; cannot be just a set of fields. Must have clearly defined ownership of what happens to the data collected.
